REQUIREMENTS FOR ENROLLMENT:
- Proof of High School Education - Diploma/Transcript, or GED certification
- Immunization Records
- TB test (ppd) - chest Xray result from a physician if TB test is positive
- Covid 19 Vaccine - or Waiver
- Enrollment Application/Agreement - (at the school)
- Social Security Card
- Driver License/State ID
- CPR/BLS Certification
- Tour of the Facility/School
- Consent for Background Check - at the school
